I have read and agree to the following liability waiver:

By checking this box I (myself or on behalf of my child or dependent) voluntarily agree to participate in the Love the Sun Art & Music Festival at the Uhrich Farm/Cresset Center on June 20th & June 21st, 2026 at the Henry H. Uhrich Farm in Johnstown, CO. By checking this box I (or on behalf of my dependent) recognize that none of: The Cresset Center, CFDI, nor Uhrich Farm LLC, has undertaken any duty or responsibility for my/their safety and I agree to assume the full responsibility for all risk of bodily injury, death, disability, and property damage as a result of participating in the Love the Sun Art and Music Festival.  I hereby recognize that these risks include severe bodily injury, illness and death.

By checking this box, I hereby state that I understand the risks involved in participating in the Love the Sun Art and Music Festival and willingly and voluntarily accept these risks (for myself or my dependent). By checking this box, I hereby surrender any right to seek reimbursement
from CFDI, The Cresset Center, Uhrich Farm LLC, Dana Burns and Barbara Calkins, and
all officers, directors, employees, agents and volunteers of the above organizations acting
officially or otherwise, from any and all claims, demands, actions or causes of action
which in any way arise from my or my dependents participation in the above described event. By checking this box, I warrant that I am not relying on any oral representations, statements, or inducement apart from the statements made on this form. By checking the box above, the parties confirm that they have read, understand and consent to the terms of this waiver agreement.
